Official name change

If you’ve officially changed your name due to marriage, nationality change or other legal reasons, you’ll be asked to provide an up-to-date identity document proving the change, such as:

• a name change certificate,
• a marriage certificate, or
• a valid ID document showing both your maiden and married name.
